What to bring
Athletic Screening forms will be mailed to homes this summer. Please bring these forms complete with parent signatures with you to the screenings. Forms must be signed by parents in order to have a screening. Copies will be available the day of the screenings as well as at www.fmchosp.com under Community & Wellness.
Please allow at least an hour for screenings and concussion baseline testing to be completed. *Athletic screenings will meet the state requirement for sport participation and include physical exam; vitals including blood pressure, height, weight and blood sugar; strength, range of motion, and posture screening and are performed by FMCH Healthcare Professionals.
Playing It Safe during COVID-19
Thank you for trusting FMCH with your athletic screening for the 2020-2021 school year. Here is what we will be doing to keep everyone safe at the screenings.
1. All participants will be screened for COVID-19 prior to the
start of their athletic screening. Anyone answering yes to the symptom questions will be asked to leave and reschedule with their primary care provider when they are fever/symptom free for 72-hours or have been tested for COVID-19 with a negative result. Students will be asked if they have any of the following symptoms that are new or worsening for them:
a. Fever or Chills b. Cough c. Shortness of breath d. Fatigue e. Muscle or body aches f. Headache g. New loss of taste or smell h. Sore throat i. Congestion or runny nose j. Nausea or vomiting k. Diarrhea 2. All participants will be required to wear a mask. 3. All staff will wear masks. Those in direct patient care will also
be wearing a face shield. 4. Please limit parent/guardian accompanying the student to one
person– we ask they also wear a mask. 5. We will properly sanitize all rooms and equipment between
students with products proven to kill COVID-19. 6. We will take extra precautions in setting up the waiting area
and screening room to meet social distancing standards of 6’ between participants.
