High School 1:1 Participation Documents for 2019/2020 School Year

Dear Fort Madison High School Students, Parents, and Guardians

We are pleased to announce the continuation of the 1-1 Chromebook initiative.

As part of our 1:1 initiative the Fort Madison Community School District (FMCSD)  provides each student who is actively enrolled at Fort Madison High School a Chromebook as well as related peripherals including case, and charger.

High School students may opt to bring their own device to school and connect to our BYOD network. The below forms do not need to be signed in a student brings his or her own device to school and does not participate in the 1:1 program.

Checklist:

  • Read the Chromebook Care and use Policy Guide 19/20
  • Print, read, and sign the High School Chromebook 1:1 Agreement 19/20 form.
  • Print, read, and fill out the Chromebook Voluntary Insurance for 2019-2020 School Year form (OPTIONAL)
  • Return filled out forms to High School Media Center by 8/31/2019
